Case management expanded to include 16 rare chronic conditions
Effective April 1, 2009, Blue Cross and Blue Shield of Florida (BCBSF) expanded its member case management services to cover 16 rare chronic conditions previously administered by AccordantCare. The chronic conditions include:
- Amyotrophic Lateral Sclerosis (ALS)
- Chrohn’s Disease
- Chronic Inflammatory Demyelinating Polyradiculoneuropathy
- Cystic Fibrosis
- Dermatomyositis
- Gaucher Disease
- Hemophilia
- Multiple Sclerosis
| - Myasthenia Gravis
- Parkinson’s Disease
- Polymyositis
- Rheumatoid Arthritis
- Scleroderma
- Seizure Disorders
- Sickle Cell Anemia
- Systemic Lupus Erythematosus

AccordantCare will continue to accept referrals and manage members enrolled in the State Employees’ PPO Plan, the Blue Cross Blue Shield Service Benefit Plan for federal employees and select administrative services only groups.
You can refer a member to the Case Management program by calling (800) 955-5692 and selecting “Option 4.”
