New national group joins BCBSF
Effective April 1, 2008, Blue Cross and Blue Shield of Florida, Inc. (BCBSF) welcomed the Independent Colleges and Universities Benefits Association (ICUBA) to BlueChoice and BlueOptions.
ICUBA members have a choice of four plans:

Participating providers may not balance bill the member for any difference between their charges and the allowed amount.
Always ask members for a copy of their ID cards at each visit and update your information as necessary. The ICUBA logo is clearly displayed on the ID cards. Include the new alpha prefix, “UBZ,” and member numbers for all claims and inquiries regarding services rendered on or after April 1, 2008. For claims and inquiries regarding services rendered prior to April 1, 2008, use the old alpha prefixes (IWF and IEE) and the old member identification cards.

Members’ yearly deductibles and out-of-pocket expenses are effective during the benefit plan year, April 1, 2008, to March 31, 2009, not the calendar year. You can verify eligibility and benefits and identify member responsibility through the Availity®1 Health Information Network. You also may call the National Strategic Account Center at (800) 664-5295 .
Admission certification and notification requests should be initiated electronically through the Availity Health Information Network. You also may use our automated telephone system, Blue Express, at (800) 397-7337, or call the UM Department at (800) 955-5692.
