Blue coverage has terminated for Circuit City members

With the liquidation of Circuit City’s U.S. operations, health insurance coverage for Circuit City employees through Blue Cross terminated as of March 31, 2009.

Please note the following:

• You should not accept ID cards with alpha prefix CCX and group number 294800.

• For services rendered prior to April 1, 2009, submit all claims to Blue Cross and Blue Shield of Florida.

• Claims for services rendered after March 31, 2009, will not be reimbursed.

Physicians and providers should review accounts receivables for any outstanding Circuit City claims and submit them immediately.
